To register you need to complete
1. GMS1 form which is available here Registration Form. Please download, complete and bring to the surgery.
2. Medical Questionnaire. At registration you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. You have the choice of collecting the forms from the surgery or downloading them
With the medical questionnaire there is an online version , which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
